Wellness Premium Discount

To earn the premium discount for 2024, employees and their covered spouse must achieve Silver Status or higher and complete a biometric screening and an online health risk assessment in the Go365 Wellness Program.
• Employees and Spouses eligible for benefits on or before 05/01/2023 are required to         meet the qualifications by 7/31/2023.
• Employees and Spouses eligible for benefits on 6/01/2023 or later are exempt from           having to meet the qualifications by 7/31/2023 and will automatically be grandfathered     into the Wellness Premium for Plan Year 2024.
• Children and Partners are not required to participate in Go365 in order for the                   employee to earn the discount.
